Charms are accessory items that only need to be present in the character's inventory to provide their magical bonuses. All charms are unique, and one can only possess one of each charm at a time. With the exception of the Sunstone of the Twin Seas and the Class Charm, they are only obtainable on Hell difficulty as part of endgame quests.
The trophy fragment system system has seen a full overhaul to the way trophy bonuses are accumulated, completely removing the high variance grinding in exchange for a one and done mini-challenge for each boss. These challenges vary per boss, but All fall into one of 3 categories:
Timer Challenges: These challenges place a trophy buff on you upon entering the zone, which only lasts a limited amount of time. You must kill the boss and upgrade the charm within the time limit to succeed. Perfection Challenges: These challenges place a trophy buff on you upon entering the area or activating the boss. You must then keep this buff on you for the duration of the boss, while avoiding specific skills or mechanics that remove it on hit. Unique Challenges: These challenges don't rely on a buff placed on your character, and thus have no penalty to dying (unless the zone has a timer lockout), and generally involve some specific mechanic in the boss itself.
Trophy challenge requirements are all readable in game by right clicking any charm with a corresponding trophy upgrade.
Only one charm of each type may be upgraded per player per game, and upgrading a charm now locks it to the class used to upgrade it. This is to force some friction in trophy trading, so that one class that has a significantly easier time with a certain trophy can't simply farm hundreds to flood the market. There will be great value in farming difficult trophies on unspecialized classes. Lab pillar bosses will continue to drop full trophies which do not apply a class lock, giving them unique value for trade or those who play multiple characters.
No system comes without its quirks however, be sure to stick to your current activity if you wish to attempt a trophy upgrade, as beginning another trophy challenge will fail the one currently active. When in doubt, make a new game for a specific trophy.

In the Level Challenge version of Tran Athulua, Philios, Priestess of Hefaetrus, Skovos, Priestess of Karcheus and Lycander, Priestess of Zerae drop the Shard of Fire, Shard of Ice and Shard of Thunder (in that order). A character of levels from 50 to 59 can combine them to create the Sunstone of the Twin Seas. Starting from level 60, this recipe also costs 2 Arcane Crystals per 10 levels.
This charm can be upgraded by transmuting it with a Scroll of Resistance, obtainable by rescuing Anya in Act V. This can be done three times, once per difficulty level.
Class Charm
This charm is obtained by killing an Ennead Necromancer in the Ennead Challenge version of Kurast 3000 BA. They drop a Heroic Torch, which will turn into the Class Charm once transmuted in the Horadric Cube.
The Nemesis in the Mirror Temple drops the Mirror of Shadows, which is used to upgrade the Class Charm in the Horadric Cube:

Dúlra Aegis
Located in four distant corners of Scosglen are the four Protected Chests, each of which drops a Scroll when destroyed. After all four Scrolls have been collected, they can be combined into the Dúlra Aegis item design.
The Corrupted Druids scattered around Scosglen have a 20% chance to drop the corresponding Tenet design and a 1% chance to drop the corresponding full Tenet. Each type of Corrupted Druid corresponds to one Tenet: Corrupted Hunter (Tenet of Conflict), Corrupted Naturist (Tenet of Harmony), Corrupted Seer (Tenet of Wisdom), Corrupted Werebear (Tenet of Restraint), Corrupted Wereowl (Tenet of Vigor) and Corrupted Werewolf (Tenet of Judgement). Each Tenet design lists the items needed to transform it into the full Tenet: two Corrupted Crystals, three Arcane Crystals, one Greater Signet of Learning, and three other ingredients:

Note that a Tenet design must be transmuted with one ingredient at a time (which will turn the colour of said ingredient on the item description from red to green), not all at once. After all requirements have been satisfied, transmuting the Tenet design with Oil of Craft and 300 thousand gold will result in the full Tenet. When the Dúlra Aegis has been combined with all six Tenets (like the previous step, it must be done one Tenet at a time), it will turn into the full charm.
The Dúlra Aegis does not provide any bonuses until transmuted with a perfect gem. When a particular gem type is transmuted with the charm, the latter will gain the first bonus listed in the following table. The gem is not consumed by the recipe. Only one bonus per gem type can be chosen; repeating the recipe will cycle between the gem's potential bonuses.

After eleven Conjuring Eyes (dropped by the Eleventh Eye) have been collected, transmuting them with the Dúlra Aegis will combine them into the Reality Piercer, which, when transmuted with the Dúlra Aegis at the Ancient Circle in Scosglen, will open a portal to The Frozen Crest, whose boss Sescheron's Destroyer drops the final component to upgrade the Dúlra Aegis.
