With the exception of the Sunstone of the Twin Seas and the Class Charm, they are only obtainable on Hell difficulty as part of endgame quests.

Sunstone of the Twin Seas
In the Level Challenge version of Tran Athulua, Philios, Priestess of Hefaetrus, Skovos, Priestess of Karcheus and Lycander, Priestess of Zerae drop the Shard of Fire, Shard of Ice and Shard of Thunder (in that order). A character of levels from 50 to 59 can combine them to create the Sunstone of the Twin Seas. Starting from level 60, this recipe also costs 2 Arcane Crystals per 10 levels.
This charm can be upgraded by transmuting it with a Scroll of Resistance, obtainable by rescuing Anya in Act V. This can be done three times, once per difficulty level.
Class Charm
This charm is obtained by killing an Ennead Necromancer in the Ennead Challenge version of Kurast 3000 BA. They drop a Heroic Torch, which will turn into the Class Charm once transmuted in the Horadric Cube.
The Nemesis in the Mirror Temple drops the Mirror of Shadows, which is used to upgrade the Class Charm in the Horadric Cube:
The Butcher's Tooth
This charm is dropped by The Butcher in the Butcher's Lair.
Bonuses are doubled when transmuted with the Mark of Victory dropped by the Heroic version of The Butcher.
Riftwalker's Eye
This charm is dropped by the Lesser Riftwalker in the Storm Oasis.
The Greater Riftwalker in the Storm Cave drops the Greater Riftwalker Essence, which can be transmuted in the Horadric Cube to upgrade the Riftwalker's Eye:

The Ascendant Riftwalker in the Storm Pit drops the Ascendant Riftwalker Essence, which can be transmuted in the Horadric Cube to upgrade the Riftwalker's Eye:

Laser Focus Crystal
This charm is dropped by the Death Projector in the Proving Grounds.
Bonuses are doubled when transmuted with the Mark of Victory dropped by the Heroic version of the Death Projector.
Optical Detector
This charm is dropped by the Infernal Machine in the Infernal Cave.
Bonuses are doubled when transmuted with the Mark of Victory dropped by the Heroic version of the Infernal Machine.
Six Angel Bag
In Island of the Sunless Sea, each Sunless Angel drops an Angel Bag when killed. Transmuting all six in the Horadric Cube fuses them into the Six Angel Bag.
The Sunless Angels also have a 1/6 chance to drop the Sunless Crystal Bird, which can be combined with the Six Angel Bag to upgrade the latter:

This charm is dropped by Tal Rasha in the past version of the Canyon of the Magi.
Horazon's Focus
This charm is dropped by the Creature of Flame in the Dimensional Sanctum.
Visions of Akarat
This charm is dropped by Akarat in the Torajan Jungles.
Sacred Worldstone Key
This charm is dropped by Bul-Kathos in the past version of the Arreat Summit.
Moon of the Spider
This charm is dropped by Lord Aldric Jitan in the Torajan Jungles.
Fool's Gold
This charm is dropped by Bull Prince Rodeo in the Corral of Corruption.
Spirit Trance Herb
This charm is dropped by Lord Primus in the Rathma Square.
The Black Road
This charm is dropped by Buyard Cholik in the Church of Dien-Ap-Sten.
Found in the Path of Enlightenment are five Great Seals, each of which releases a Spirit boss and minions when destroyed. Combining the Great Skull, Great Rainbow Stone, Great Topaz, Great Ruby and Great Sapphire dropped by these Spirits with the Black Road adds the following bonus:
Right-clicking on the Black Road creates a red portal to Fauztinville.
Legacy of Blood
This charm is dropped by Bartuc in the Chamber of Blood.
Trophy Challenge: Kill Bartuc and cube the charm within 15 seconds of entering.
Idol of Vanity
This charm is dropped by The Prophet in the Cathedral of Vanity.
Trophy Challenge: Kill the Prophet without being struck by the Veil Piercer's lightning.
Azmodan's Heart
This charm is dropped by Azmodan in the Heart of Sin.
Trophy Challenge: Kill Azmodan without being hit by a Molten Boulder from a Flameblood.
Hammer of the Taan Judges
This charm has a 1/3 chance to be dropped by an Ennead Necromancer in Kurast 3000 BA.
Zakarum's Ear
This charm has a 1/6 chance to be dropped by a Zakarum's Avatar in the Library of Fate.

This charm is dropped by Imperius in the Halls of Valor and Malthael in the Pools of Wisdom.
It can be upgraded by killing both Imperius and Malthael within a minute, then transmuting it in the Horadric Cube.

Trophy Challenge: Kill both Imperius and Malthael within 60 seconds of the hooded figure disappearing. Cube with both buffs on your character.

This charm is dropped by Manoruk at the Court of the Damned.
There are multiple versions of this charm which can drop, With high or low values. All of which scale with bonus damage to undead and demons. They are as follows:
30/36% added as Weapon Physical damage
6/8% added as Innate Elemental Damage
15/18% added as Physical/Magical Spell Damage
15/18% added as Fire/Lightning Spell Damage
15/18% added as Cold/Poison Spell damage

Lylia's Curse
This charm is dropped by Lilith in Toraja.
Umbaru Treasure
This charm is dropped by Zagraal's Shadow in Teganze.
It can be upgraded by transmuting with essences, which have a chance to be dropped by Umbaru Spirits and Zagraal's Shadow.

Corrupted Wormhole and Cycles
The Corrupted Wormhole is an empty charm dropped by Lucion in the Bastion of the Triune.
Trophy Challenge: Kill Lucien without being struck by Malic's burn missiles.
While the Corrupted Wormhole does not come with any bonuses, it can be customised by cycles, which can also only be found in the Triune, and add bonuses to the Corrupted Wormhole when transmuted with it. There are 31 normal cycles and one special Golden Cycle, which is very rare and can only be applied once.

The four Star Charts are dropped by the Corrupted Assassin, Abyssal Shrine bosses, Navigator and Unstable Anomaly; the five Dream Fragments are dropped by the Corrupted Amazon, Corrupted Barbarian, Corrupted Necromancer, Gharbad the Weak and Phoboss, Nephalem's Bane in that order.
Dúlra Aegis
Located in four distant corners of Scosglen are the four Protected Chests, each of which drops a Scroll when destroyed. After all four Scrolls have been collected, they can be combined into the Dúlra Aegis item design.
The Corrupted Druids scattered around Scosglen have a 20% chance to drop the corresponding Tenet design and a 1% chance to drop the corresponding full Tenet. Each type of Corrupted Druid corresponds to one Tenet: Corrupted Hunter (Tenet of Conflict), Corrupted Naturist (Tenet of Harmony), Corrupted Seer (Tenet of Wisdom), Corrupted Werebear (Tenet of Restraint), Corrupted Wereowl (Tenet of Vigor) and Corrupted Werewolf (Tenet of Judgement). Each Tenet design lists the items needed to transform it into the full Tenet: two Corrupted Crystals, three Arcane Crystals, one Greater Signet of Learning, and three other ingredients:

Note that a Tenet design must be transmuted with one ingredient at a time (which will turn the colour of said ingredient on the item description from red to green), not all at once. After all requirements have been satisfied, transmuting the Tenet design with Oil of Craft and 300 thousand gold will result in the full Tenet. When the Dúlra Aegis has been combined with all six Tenets (like the previous step, it must be done one Tenet at a time), it will turn into the full charm.
The Dúlra Aegis does not provide any bonuses until transmuted with a perfect gem. When a particular gem type is transmuted with the charm, the latter will gain the first bonus listed in the following table. The gem is not consumed by the recipe. Only one bonus per gem type can be chosen; repeating the recipe will cycle between the gem's potential bonuses.

After eleven Conjuring Eyes (dropped by the Eleventh Eye) have been collected, transmuting them with the Dúlra Aegis will combine them into the Reality Piercer, which, when transmuted with the Dúlra Aegis at the Ancient Circle in Scosglen, will open a portal to The Frozen Crest, whose boss Sescheron's Destroyer drops the final component to upgrade the Dúlra Aegis.
